In the twenty-first century, minimally invasive surgical (MIS) techniques have become the standard of surgical care for many patients. This approach involves the use of an endoscope by a surgeon to allow him/her to effectively treat a variety of conditions. An endoscope is a thin tube with a light and tiny viewing device.
In the field of Gynecology, hysteroscopy is a minimally invasive intervention that can be used to accurately examine the inside of the vagina, cervix and uterus to aid in the diagnosis and effectively treat diseases or pathologies. It is a see and treat procedure.
In the Philippines, there is a growing trend towards ambulatory health care setting, pioneered by Clinica Manila, in the management of common health problems. With the advent of small caliber endoscopes, hysteroscopy is one such procedure that can easily be performed in such settings to diagnose and treat minor pathologies affecting women and allow her to return home same day. Local or conscious sedation anesthesia may be given during hysteroscopy even in an ambulatory setting depending on the suitability of the patient and the minor hysteroscopic procedure to be performed.
Indications for ambulatory hysteroscopy:
I. Diagnostic hysteroscopy
a. Investigation of abnormal uterine bleeding among pre-menopausal and menopausal women
b. Investigation of infertility
II. Minor operative hysteroscopy
a. Endometrial polyp removal
b. Small submucous myoma removal
c. Endocervical lesion removal
d. Dissection of loose intrauterine adhesions
e. Removal of intrauterine devices with missing threads
f. Removal of foreign objects
Hysteroscopy is considered a safe procedure with minimal complications in the hands of certified surgeons. Ambulatory hysteroscopy is the gold standard for the investigation of the uterine cavity pathologies and the performance of minor operative procedures with less risk and low cost.
